The LPN will provide high-quality nursing care to patients across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. This role involves delivering patient-centered care, assisting with medical procedures, and supporting the overall health and well-being of diverse patient populations. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to maintaining the highest standards of patient safety and confidentiality. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and medication administration. Assist with medical procedures such as IV infusions, catheterizations, wound care, and sterile techniques. Manage electronic health records (EHR/EHR systems) accurately, documenting patient information in compliance with HIPAA and confidentiality policies. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or developmental disabilities. Support acute pain management, hospice care, and emergency response efforts in collaboration with interdisciplinary teams. Conduct triage assessments to prioritize patient needs in urgent or emergency situations. Educate patients and their families on health management strategies, medication use, and post-discharge care plans. Collaborate with physicians, case managers, and other healthcare professionals to develop individualized care plans. Maintain compliance with long-term care regulations and facility policies while ensuring quality patient care. Participate in ongoing training related to ICD coding (International Classification of Diseases), infection control, and clinical best practices.
Requirements:
Minimum 2-year LPN Experience Required BLS Housing available
