Job Description

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) with a valid US driver's license to join our healthcare team. This vital role involves providing high-quality patient care across various settings, including long-term care facilities, assisted living communities, hospitals, and home health environments. The ideal candidate will possess a strong clinical background, excellent communication skills, and a commitment to enhancing patient well-being through attentive and professional nursing practices. This position offers an opportunity to work in dynamic healthcare settings while making a meaningful difference in patients' lives. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control or chronic conditions. Assist with diagnostic evaluations, specimen collection, and processing to support accurate medical diagnoses. Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, or other memory impairments, ensuring safety and comfort. Support patients with activities of daily living (ADLs), including tube feeding, catheter care, and ventilator management when necessary. Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with long-term care regulations and best practices. Maintain strict adherence to HIPAA regulations and clinical confidentiality policies while documenting all patient interactions accurately in EMR/EHR systems. Participate in discharge planning, case management, and patient education to promote health coaching and self-care strategies. Experience Prior experience working with diverse patient populations across settings such as skilled nursing facilities, hospitals (including ICU, emergency room, trauma centers), outpatient clinics, or home care environments. Demonstrated proficiency with EMR systems like Epic, Cerner, Athenahealth, or eClinicalWorks for medical documentation and records management. Critical care experience including ICU or Level I/II trauma center exposure is highly desirable. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding, infection control procedures, and medical terminology is essential. Experience managing acute pain cases, infectious diseases, hospice care, or palliative services is advantageous. Familiarity with working with individuals with disabilities or developmental delays enhances the quality of patient-centered care delivery. Strong physiology knowledge combined with skills in physical examinations, venipuncture (phlebotomy), sterile technique, and aseptic procedures supports effective clinical practice.
