$1,936 / 32 hours Required credentials Current Minnesota LPN/LVN license BLS (Basic Life Support) certification Long-term care experience preferred Core responsibilities Provide direct nursing care and clinical leadership in a long-term care household, modeling professional nursing standards and person-centered/resident-directed care. Observe and assess residents' conditions, coordinate with other services to ensure continuity and quality of care. Implement and follow clinical policies and procedures to ensure safe, sanitary, and efficient practices (including medication administration, documentation, RAI/charting, safety, infection control, and resident rights). Ensure clinical operations comply with federal, state, and local regulations and the Minnesota Nurse Practice Act. Participate in orientation, training, competency evaluation, coaching, education, and performance appraisal of staff. Collaborate with interdisciplinary team members, physicians, and other medical personnel to develop and implement resident care plans. Communicate and enforce household clinical policies and procedures; foster collaboration and problem solving among staff and with residents/families. Support efficient use of household resources and contribute to fiscal responsibility. Promote a welcoming, home-like environment and provide emotional, psychological, and spiritual support to residents as appropriate.
