Job Overview Join our dedicated team as a Long-Term Care Facility Registered Nurse (RN) or Licensed Practical Nurse (LPN), where your expertise will directly impact the quality of life for residents in a compassionate, dynamic environment. This role offers an exciting opportunity to provide comprehensive patient care, coordinate treatment plans, and support individuals with diverse healthcare needs, including those with complex conditions such as dementia, Alzheimer's disease, and chronic illnesses. Your energetic approach and critical thinking will help foster a safe, nurturing space for our residents to thrive. Responsibilities Deliver personalized nursing care to residents in accordance with physician orders, facility policies, and regulatory standards. Conduct thorough patient assessments, including vital signs, physical examinations, and detailed medical histories. Administer medications, injections, IV infusions, and treatments such as tube feeding and catheter care with precision and adherence to safety protocols. Monitor residents' health status continuously, recognizing signs of acute pain, infection, or deterioration requiring immediate intervention. Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of services like hospice care or specialized memory care programs. Maintain accurate medical documentation using EMR (Electronic Medical Record) systems such as Epic or Cerner; ensure HIPAA compliance at all times. Educate residents and their families on health coaching topics including medication management, nutrition, and chronic disease management. Qualifications Valid RN or LPN license with current state certification; additional certifications in Basic Life Support (BLS) required. Demonstrated experience working in long-term care facilities, nursing homes, assisted living environments, or similar settings. Familiarity with EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks; proficiency in
ICD-9/10
coding is preferred. Experience with complex patient populations including geriatrics, dementia/dementia care (Alzheimer's), behavioral health issues, and individuals with developmental disabilities. Knowledge of infection control protocols and aseptic techniques; experience with IV insertion and sterile procedures is highly desirable. Strong assessment skills in physiology and anatomy; ability to perform diagnostic evaluations and manage acute pain or trauma cases effectively. Excellent communication skills for triage situations, family interactions, and interdisciplinary collaboration; ability to work efficiently in fast-paced environments like hospital medicine or emergency settings is a plus.
