Job Description
Staff Nurse (LPN) Living Well Disability Services

Planning:
Follow medication and health care policies and procedures as determined by the organization. Perform ongoing evaluation of each person's status to assure appropriate health care is being provided. Assist in the development of health care profiles and nursing/health care assessments as needed.Organization:
Effectively train and mentor staff to develop the skills necessary to assist and support people to achieve desired health care outcomes including the use of medications and treatments. Ensure staff understand the specifics of each person supported's health care plan and needs, providing additional training as needed.Communication:
Communicate effectively and cooperatively with a variety of people including providers, interdisciplinary teams, and organizational teams. Ability to answer health and medication questions about people supported.Teamwork:
Work as a team members to support all people to help them maintain the health they need to access their communities, act on potential, and meet their desired outcomes. Longevity of staff is a strength at Living Well, with many of our nurses having started with us as Direct Support Professionals working in the homes and moving into nursing positions within our organization.
