The ideal candidate will possess a strong foundation in patient care, with extensive experience in various clinical settings. As an LPN, you will provide compassionate, high-quality nursing support by performing patient assessments, administering medications, and assisting with medical procedures. Your role is vital in ensuring the well-being of our patients and supporting the overall operations of our healthcare facility. This position offers an opportunity to make a meaningful difference in patients' lives while working within a dynamic and supportive environment. Duties Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories, utilizing physiology knowledge and medical terminology. Administer medications and injections accurately following HIPAA guidelines and established protocols, including IV infusion and catheter care. Assist physicians during medical procedures such as catheterization, suturing, or spinal tap, ensuring aseptic technique is maintained at all times. Manage patient documentation using EMR (Electronic Medical Records) systems such as Cerner or Athenahealth to ensure accurate medical records and compliance with healthcare standards. Provide specialized care for patients with complex needs such as Alzheimer's disease, dementia, or those requiring hospice or palliative care. Support discharge planning by coordinating follow-up care, educating patients and families on medication administration, nutrition, and post-acute care procedures. Collaborate with interdisciplinary teams in settings like long-term care, assisted living, hospitals—including Level I Trauma Centers—and outpatient clinics to optimize patient outcomes. Participate in infection control practices and maintain sterile processing standards to uphold safety across all clinical activities. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) with current certification to practice in the relevant jurisdiction. Demonstrated experience working in diverse healthcare environments such as hospitals (including ICU and emergency room), nursing homes, or home & community care settings. Proficiency with EMR/EHR systems like Cerner or Athenahealth for accurate medical documentation and coding (ICD-9/ICD-10). Critical care experience preferred, including ICU or Level II trauma center exposure; experience with acute pain management and ventilator support is advantageous. Knowledge of medical imaging procedures, phlebotomy, tube feeding techniques, and IV insertion skills required. Strong understanding of HIPAA regulations, infection control protocols, and medication administration standards. Excellent patient assessment skills coupled with physiology knowledge to evaluate symptoms effectively across various age groups—including pediatrics and neonatal ventilator management. Ability to work effectively within multidisciplinary teams while demonstrating compassion for individuals with disabilities or cognitive impairments such as Alzheimer's or dementia.
