LPN Private Home Care - Columbia MO Columbia, MO Job Details Part-time | Full-time $16 - $21 an hour 1 hour ago Qualifications LPN Stakeholder relationship building Full Job Description Private Home Care is seeking a compassionate and organized Licensed Practical Nurse (LPN) to join our team in a full-time role supporting one of our valued partner facilities. This position is ideal for an LPN who enjoys building relationships, leading caregivers, and making a meaningful impact on both clients and staff.
Responsibilities:
Oversee and support Private Home Care caregivers within the partner facility Create and manage caregiver schedules Build and foster strong client and family relationships Complete new client assessments and reassessments Develop and update individualized care plans Collaborate with facility staff and the PHC team to ensure exceptional care and communication Provide ongoing guidance and support to caregivers to maintain quality standards Other duties as assigned
Qualifications:
Current LPN license in good standing 3+ years of experience in professional setting Strong communication and organizational skills Experience in home care, senior care, or long-term care preferred Leadership experience is a plus Passion for providing compassionate, client-centered care
