- Licensed Practical Nurse LPN
- , you'll contribute to our success in the following ways: + Deliver in-home nursing care aligned with patient care plans + Review and follow Plan of Care prior to each visit + Communicate patient updates, concerns, and changes in condition + Assist with medication management and documentation + Educate patients and families on self-care techniques + Identify safety concerns and help prevent hospitalizations + Collaborate with RN Case Managers, physicians, and care team •What is required?
- + Current LPN/LVN license, in the state of Missouri + Graduate of an accredited nursing program + At least 1 year of experience in acute care ( home care, home health or hospice preferred) + Strong communication and interpersonal skills + Ability to manage time effectively and stay organized in the field + Knowledge of healthcare regulations and patient care standards + Compassionate, patient-first mindset + Reliable personal transportation (travel required ~50%) + Ability to meet health screening and compliance requirements You will report to the Clinical Manager or Branch Director for Personal Care Services.
