Licensed Practical Nurse

Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in the home healthcare setting. The LPN will work closely with registered nurses and other healthcare professionals to ensure optimal patient outcomes through effective assessment, planning, and implementation of care.

Two shifts:

Day shift: 10:30am-7:00pm,

Night Shift:

9:30pm-5:00am Duties Administer medications and treatments as prescribed by physicians. Monitor patients' vital signs and report any significant changes to the healthcare team. Experience working with children and babies. Assist in the development and implementation of individualized care plans. Perform patient assessments, including physical examinations and health history intake. Provide support for patients with chronic conditions, including Alzheimer's care, dementia care, and senior care. Engage in triage and emergency care situations as needed. Maintain accurate medical documentation in compliance with HIPAA regulations. Collaborate with interdisciplinary teams to coordinate patient care. Educate patients and their families on health management and wellness strategies. Ensure adherence to infection control protocols and aseptic techniques. Ability to work independently and as part of a team. Knowledge of healthcare regulations and patient care standards. Experience Candidates should possess experience in one or more of the following areas: Long-term care or assisted living environments Home care or hospice care Familiarity with

ICD-9/ICD-10

coding and medical terminology Experience working with individuals with developmental disabilities Knowledge of pain management techniques and acute pain management Proficiency in basic life support (BLS), phlebotomy, IV insertion, catheterization, and medication administration.

Pay Range:

$35 per hr Pay Frequency (Semi-Monthly)
