  • Full-time Job Overview We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team 7a-7p and every other weekend.
The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. As an LPN, you will play a crucial role in supporting patients' health and well-being through effective nursing practices and collaboration with healthcare professionals. Responsibilities Provide direct patient care, including administering medications and treatments as prescribed. Conduct patient assessments and monitor vital signs to ensure optimal health outcomes. Assist with daily living activities for patients, including personal hygiene and mobility support. Document patient care activities accurately in electronic health records (EHR) systems. Collaborate with registered nurses and physicians to develop and implement individualized care plans. Educate patients and their families about health management and disease prevention. Maintain a clean and safe environment for patients, adhering to infection control protocols. Participate in discharge planning and coordinate follow-up care as needed. Qualifications Valid LPN license in the state of practice. Experience in critical care, long-term care, or acute care settings is preferred. Familiarity with medical terminology, ICD coding (ICD-9 & ICD-10), and EMR systems such as Epic or Cerner. Knowledge of patient assessment techniques and basic life support (BLS) certification is required. Experience working with diverse populations, including those with developmental disabilities, dementia, or Alzheimer's care. Strong communication skills to effectively interact with patients, families, and healthcare teams. Ability to perform procedures such as IV insertion, phlebotomy, catheterization, and medication administration. Understanding of geriatric care principles and familiarity with hospice or palliative care practices is a plus.
