We are seeking a detail-oriented and organized Licensed Practical Nurse (LPN) to join our team as a Quality Assurance Assistant in a home health setting. This role supports compliance, documentation accuracy, and quality improvement initiatives to ensure exceptional patient care and adherence to regulatory standards. Key Responsibilities Review clinical documentation for accuracy, completeness, and compliance with state and federal home health regulations Assist with quality assurance audits and performance improvement activities Track and report quality metrics, trends, and outcomes Collaborate with clinical staff to ensure proper documentation and timely corrections Support survey readiness and accreditation processes Maintain patient confidentiality and adhere to HIPAA guidelines Assist in policy and procedure updates as needed Qualifications Current, active LPN license in Missouri Minimum of 2 years of experience in home health, hospice, or related field preferred Knowledge of home health regulations (e.g., Medicare Conditions of Participation) Strong attention to detail and organizational skills Proficiency in electronic medical records (EMR) systems Excellent communication and teamwork skills Preferred Skills Experience with OASIS documentation and review Familiarity with quality improvement processes Ability to analyze data and identify trends
