Job Description

Our facility offers education reimbursement up to $2000 to newly hired LPNs or RNs who have just graduated (within 6 months) in good standing from a state approved LPN training program or RN program! Ask for more information when applying. Job Summary Join our dedicated healthcare team as a Licensed Practical Nurse (LPN) working Monday through Friday from 7 AM to 3 PM. In this vital role, you will provide compassionate, high-quality patient care across various settings, including long-term care, assisted living, and acute care environments. Your expertise will support patient recovery, comfort, and safety through skilled assessments and personalized interventions. Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individual care needs. Administer medications and injections accurately following established protocols while ensuring compliance with HIPAA and medical documentation standards. Manage patient care plans by collaborating with physicians, case managers, and other healthcare professionals to optimize health outcomes. Document all patient interactions thoroughly using EMR (Electronic Medical Records) or EHR (Electronic Health Records) . Provide education on health coaching topics such as nutrition, medication administration, dementia care (including Alzheimer's and memory care), and discharge planning for patients and their families. Experience Proven experience working as an LPN in diverse healthcare settings such assisted living and LTC. Experience working with individuals with disabilities or developmental delays in settings like assisted living or home & community care is advantageous. Knowledge of physiology, anatomy, pharmacology principles related to medication administration and patient assessment techniques. Ability to operate medical imaging devices or perform specimen collection/processing when required. Strong understanding of HIPAA regulations along with proficiency in phlebotomy, tube feeding management, and basic life support (BLS). $25.00 - $27.50 per hour
