•We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care during the 7AM to 7PM shift. The ideal candidate will possess a strong foundation in nursing practices, with experience in diverse healthcare settings, including acute and long-term care environments. As an integral part of our healthcare team, you will deliver comprehensive nursing services, support patient assessments, and ensure adherence to safety and regulatory standards. Your expertise will contribute to improving patient outcomes and fostering a caring, professional environment. Duties Perform thorough patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems Administer medications and injections accurately following prescribed protocols, including IV infusions and medication administration techniques. Assist with complex procedures such as catheterization, tube feeding Monitor patients' conditions continuously, recognizing signs of acute pain or deterioration requiring immediate intervention or triage. Collaborate with interdisciplinary teams in settings like hospitals, nursing homes, assisted living facilities, or home care to develop and implement individualized care plans. Maintain detailed medical documentation adhering to HIPAA regulations and coding standards such as

for accurate record-keeping. Support infection control practices and ensure compliance with safety protocols across all patient interactions. Provide specialized care for individuals with developmental disabilities, dementia (including Alzheimer's and memory care), or those requiring hospice or palliative services. Qualifications Valid licensure as a Licensed Practical Nurse (LPN) with current state certification. Proven experience in diverse healthcare environments such as hospital medicine, emergency room, ICU, medical-surgical units, or long-term care facilities. Proficiency in EMR/EHR systems Strong knowledge of physiology, anatomy, pharmacology, and medical terminology relevant to patient assessment and treatment. Familiarity with critical care procedures such as ventilator management or airway management is desirable. Ability to work effectively with individuals with disabilities or complex health conditions like dementia or infectious diseases. Excellent communication skills for patient education, team collaboration, and documentation accuracy. Knowledge of HIPAA regulations ensuring confidentiality of patient information at all times.
