In this vital role, you will provide compassionate, efficient nursing support across various settings, including long-term care facilities, hospitals, outpatient clinics, and community health environments. Your dedication will help ensure patients receive high-quality care through accurate assessments, medication administration, and personalized patient support. Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and evaluating patient conditions to inform care plans. Administer medications and injections accurately Manage patient documentation using electronic health record (EHR) systems such as PCC to ensure precise medical records and compliance with HIPAA regulations. Support patients with complex needs such as dementia or Alzheimer's disease through memory care strategies and behavioral health interventions. Educate patients and families on health coaching topics including medication management, nutrition, discharge planning, and chronic disease management. Assist in emergency situations by managing airway control or basic life support (BLS) procedures as needed. Ensure compliance with all healthcare standards including infection control protocols and documentation accuracy for medical coding (ICD-10/ICD-9). Proven experience working with diverse patient populations across settings such as long-term care, assisted living, hospital medicine, or home & community care. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and treatment planning. Skilled in using PCC for documentation and order entry. Excellent communication skills for triage assessments, patient education, and interdisciplinary collaboration. Knowledge of ICD coding (ICD-10/ICD-9), CPT coding for procedures, and medical records management to ensure accurate billing and documentation compliance. Compassionate approach to working with individuals with disabilities or behavioral health needs; experience in dementia or Alzheimer's care is a plus. Ability to adapt quickly in high-pressure situations while maintaining attention to detail in all aspects of patient care.
