Job Description

Job Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA) to join our healthcare team. Monday - Thursday 8:00am- 4:30pm and Fridays 8:00am-12:00pm. We are closed on all major Holidays. No weekends. This role involves providing high-quality patient care across various settings, including acute care, long-term care, outpatient clinics, and community health environments. The ideal candidate will possess a strong foundation in medical procedures, excellent patient assessment skills, and experience working with diverse patient populations, including seniors and individuals with disabilities. Responsibilities Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation Administer medications and injections accurately following physician orders and safety protocols Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, hospice care, or acute pain management Support discharge planning and coordinate follow-up care for outpatient and inpatient populations Document all patient interactions thoroughly within EMR/EHR systems like Epic or Aprima Educate patients and families on health management, medication adherence, and post-care instructions Ensure compliance with HIPAA regulations and infection control standards at all times Refill medications Skills Proven experience in primary care or home & community care Proficiency with EMR/EHR systems including Epic, Aprima Strong knowledge of medical terminology Skilled in patient assessment techniques including vital signs measurement and physical examinations Ability to perform venipuncture, phlebotomy Experience administering injections and managing ventilators or respiratory support devices Excellent communication skills for working with patients Knowledge of infection control practices and aseptic techniques to prevent healthcare-associated infections Ability to work efficiently under pressure in high-acuity environments This position requires candidates to pass a background check and drug screening prior to employment. Candidates should demonstrate a commitment to delivering compassionate patient care while adhering to all safety standards.
