- Designed to support your well-being and future What You'll Do Deliver Compassionate Hospice Care Provide direct patient care in accordance with the state Nurse Practice Act, including end-of-life care Implement and support individualized plans of care under RN supervision Deliver effective pain and symptom management, evaluating patient response to treatment Educate patients and caregivers on disease progression, comfort measures, and care needs Maintain a safe, clean, and supportive care environment Collaborate & Communicate Document care accurately and timely in the EMR Partner with physicians and interdisciplinary team members (RN, HHA, Social Work, Chaplain, etc.
