Licensed Practical Nurse

Overview We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. Responsibilities Deliver comprehensive patient care by monitoring vital signs, administering medications, and performing routine physical examinations. Maintain accurate medical documentation within electronic health record (EHR) systems, ensuring compliance with HIPAA regulations. Conduct patient assessments and triage to prioritize urgent needs and coordinate appropriate interventions. Support patients with complex needs including dementia, Alzheimer's disease, memory care, and behavioral health challenges. Ensure infection control protocols are strictly followed to prevent the spread of infectious diseases within healthcare environments such as hospitals or clinics. Provide patient education on medication administration, nutrition, pain management, and health coaching tailored to individual needs. NOC shift 6PM-6AM Requirements Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Proven experience in diverse clinical settings such as long-term care facilities. Knowledge of medical terminology, anatomy, physiology, and diagnostic evaluation techniques. Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality at all times. Excellent communication skills for effective patient assessment and collaboration with healthcare teams.
