Job Description
Job Summary The
PACT LPN
provides practical nursing care to adult and elderly Veterans in Primary Care using the Patient Aligned Care Team (PACT) model. You will support chronic disease management, health promotion, and patient education while working closely with the RN Care Manager, Primary Care Provider, and other team members. Responsibilities Provide a full range of practical nursing care in Primary Care, including assessments within LPN scope, medication administration, treatments, and patient/family education. Prepare and administer oral, topical, subcutaneous, intramuscular, and IV medications per policy; observe and document patient response, reporting changes to RN/MD. Assist with complex diagnostic tests and procedures, including preparing the patient, handling specialized equipment, and monitoring condition before, during, and after procedures. Recognize urgent/emergent situations, seek assistance, and initiate emergency interventions (e.g., BLS) as directed. Document care, assessments, treatments, and patient education in VistA/CPRS; complete clinical reminders and accurate encounters. Participate in PACT activities such as daily huddles, triage support, pre‑visit calls, and panel management to support access and care coordination. Serve as a resource and preceptor for less experienced LPNs or NAs as needed. Experience U.S. citizenship and graduation from an approved school of practical/vocational nursing. Current, unrestricted LPN/LVN license. BLS certification. At least one year of LPN experience at the GS‑5 level (or equivalent), meeting all performance requirements. Demonstrated skill in practical nursing care, medication administration, use of electronic health records, and working with diverse adult Veteran populations. Schedule, Travel and work environment Intermittent/PRN schedule based on clinic needs; hours may vary. Travel within Montana 25-50% of the time; when not traveling, you work at your home Primary Care site. Frequent standing, walking, bending, and lifting; exposure to patients with complex medical conditions and to infectious materials with PPE provided.Pay:
