Overview:
We are seeking a compassionate and skilled Home Care Nurse to join our team. As a Home Care Nurse, you will provide personalized skilled nursing care to members in the comfort of their own homes. This is a rewarding opportunity to make a positive impact on the lives of individuals in need.
Duties:
  • Conduct comprehensive assessments of health status
  • Develop and implement individualized care plans
  • Observes, identifies, and documents any signs and symptoms of changes in the client's physical, emotional/mental/social condition, reactions and medications, and environmental safety factors.
  • Administer medications and treatments as prescribed
  • Maintains accurate and timely records and observes the confidentiality of the medical record
  • Educate members and their families on proper healthcare procedures and disease management
  • Collaborate with other healthcare professionals to ensure coordinated and effective care
Skills:
  • Strong knowledge of human anatomy and physiology
  • Competence in medication administration
  • Adhere to professional/industry standards of practice/care
Requirements:
  • Valid nursing license in MT
  • Excellent communication, documentation, and interpersonal skills
  • Ability to work independently and prioritize tasks effectively
  • Current driver's license and auto insurance
