Carolina Outreach is a group of highly trained behavioral health professionals dedicated to promoting the emotional and physical well-being of our community in a safe and respectful environment. We provide a comprehensive and integrative array of behavioral health services and supports for people of all ages with varying levels of need utilizing an equitable and inclusive approach. We focus on helping our clients tap into and develop their inherent strengths with the goal of supporting the highest quality of life possible for all we serve. About the Role Monitor and oversee all medical/clinical aspects of services delivered. Provide medication monitoring and documentation. Deliver psychopharmacological and medical services within your scope of practice. Collaborate with ACT team members to monitor clinical status and response to treatment. Conduct crisis assessments, symptom management, and intervention planning. Provide individual counseling, substance abuse treatment, and psycho-educational support. Assist in creating transition plans when clients move in and out of services. Act as a consultant to primary healthcare providers and community medical staff. Ensure proper documentation, including treatment plans and progress notes. Be available for crisis response and support to the ACT team on a 24/7/365 basis. Compensation:
$25.02 - $32.13 Final compensation is determined by factors such as education, licensure level, experience and other qualifications. Education/Licensure :
Active LPN license in North Carolina.
Experience :
