Job Description

Home Health LPN Location:
Lake Norman and Surrounding Areas/Gaston County We are seeking a skilled and compassionate Home Health LPN to join our team. The Home Health LPN will be responsible for providing nursing care to patients in their homes, ensuring their comfort and safety, under the direction of a physician. The ideal candidate will have excellent communication and interpersonal skills, as well as a strong commitment to patient care. PHC Home Health is a rapidly growing full-service Home Health Agency looking for an energetic and dedicated team member who is willing to learn new skills. This is a fast-paced role in a growing company working in a very friendly and dynamic team. We are growing quickly and looking for dedicated Home Health professionals to staff Lake Norman and Surrounding areas and Gaston County to provide excellent patient care.
Responsibilities:
Provide nursing care to patients in their homes; including treatments, diagnostic procedures, and patient education based on the plan of care and physician's orders Administer medications and treatments as prescribed by the physician Monitor patients' vital signs and report any changes to the physician Fulfill duties as outlined in the patients' care plans and suggest any changes needed to the supervisor and physician Observe signs and symptoms, reactions to treatment, drug response/interactions, and changes in the patient's condition; with appropriate follow-up in care and reporting to supervisor or physician as needed Helps coordinate care, as needed with patient/responsible party, physician, other disciplines, and or internal and external resources Communicate with patients' families and caregivers Maintain accurate and up-to-date patient records Collaborate with other healthcare professionals to ensure the best possible patient care Participates in multi-disciplinary team meetings Participates in learning and job-enhancement activities as directed by the supervisor Participate in On-Call rotation
Requirements:
Current LPN license in the state of North Carolina Minimum of 1 year of experience in home health or a related field Excellent communication and interpersonal skills Excellent time management and organizational skills Great technology skills (use iPad/Laptop to record visit notes) Strong commitment to patient care Ability to work independently and as part of a team Valid driver's license and reliable transportation
