RN/LPN MDS
Job Summary:
We are seeking a dedicated and compassionate
RN/LPN MDS
to join our team at Pine Acres Center for Nursing and Rehabilitation in Lexington, North Carolina. As a member of our team, you will play a vital role in providing high-quality, resident-centered care to our patients. Responsibilities:
Assess patient care needs and develop individualized care plans Coordinate with interdisciplinary team to ensure comprehensive care Monitor and report patient progress and concerns to the healthcare team Implement and update Minimum Data Set (MDS) assessments Maintain accurate and timely documentation Participate in quality improvement initiatives to enhance patient care
Requirements:
Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) in the state of North Carolina Minimum 0 years of experience in long-term care or acute care setting Strong communication and interpersonal skills Ability to work effectively in a fast-paced environment Commitment to providing high-quality, resident-centered care
