[Full-Time/Part-Time/PRN] Job Overview Nowell Care at Home, LLC is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our growing team. Unlike clinical or facility-based roles, this position focuses on providing personalized, one-on-one nursing care within the comfort of our clients' homes. You will play a critical role in helping seniors and individuals with disabilities maintain their independence while receiving high-quality medical oversight.

Key Responsibilities One-on-One Patient Care:

Deliver tailored nursing interventions based on individual care plans, focusing on the unique needs of the home environment.

Clinical Monitoring:

Monitor vital signs, perform physical assessments, and report any significant changes in patient status to the Registered Nurse (RN) or physician.

Medication Management:

Administer medications (oral, topical, or injectable) and assist with med-box setups and refills.

Specialized Treatments:

Perform wound care, catheter management, and basic respiratory support as prescribed.

Chronic Disease Education:

Empower patients and their families by providing education on nutrition, medication safety, and fall prevention.

Documentation:

Maintain accurate, timely electronic health records (EHR) to ensure compliance with North Carolina state regulations and Medicaid/Medicare guidelines.

Care Coordination:

Work closely with our team of caregivers and administrative staff to ensure a seamless "Care at Home" experience.

Requirements License:

Active, unencumbered North Carolina LPN license (or Compact State license).

Experience:

Minimum of 1 year of nursing experience; prior experience in home health or private duty nursing is strongly preferred.

Skills:

Proficiency in wound care, medication administration, and patient assessment. Familiarity with electronic charting is a plus.

Certifications:

Current BLS/CPR certification.

Reliability:

Valid driver's license, reliable transportation, and proof of auto insurance (travel to client homes is required).

Soft Skills:
