Licensed Practical Nurse - Long Term Care Job Overview We are seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care team in a skilled nursing environment. The ideal candidate will have previous LTC experience, strong clinical skills, and a commitment to providing exceptional resident-centered care.

ASAP Requirements Active and unrestricted LPN license Previous Long-Term Care or Skilled Nursing experience required Strong medication administration and resident assessment skills Ability to work independently and as part of a multidisciplinary team Excellent communication and documentation skills Current BLS certification preferred Positive employment references and a strong history of contract performance preferred Responsibilities Provide direct nursing care to residents in accordance with individualized care plans Administer medications, treatments, and nursing interventions as prescribed Monitor resident conditions and report significant changes to the healthcare team Maintain accurate and timely clinical documentation Assist with admissions, transfers, and discharge processes as needed Collaborate with physicians, nurses, CNAs, and other healthcare professionals to ensure quality resident care Respond promptly to resident needs and emergency situations Ensure compliance with infection control practices, facility policies, and regulatory standards Promote resident comfort, safety, dignity, and overall well-being Additional Information Immediate start available Skilled Nursing / Long-Term Care setting On-site housing may be available for travelers Additional shifts may be available based on facility needs, though not guaranteed Candidates should be flexible and available for night shift scheduling Availability will be confirmed prior to submission

$35.00 - $45.00 per hour
