(LPN)PALLIATIVE CARE COORDINATOR - HOSPICE SERVICES

KEY RESPONSIBILITIES
- Responsible for taking and processing Palliative Care referrals.
- Provide support to the Palliative Care Nurse Practitioner and Medical Director.
- Will schedule visits to support goals of care as ordered by MD.
- Coordinate and communicate patient care goals between all providers.
- Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.
- Ensure communication of assessment and reassessment findings to attending physician.
- Complete all documentation accurately, timely, and in accordance with company policy and procedure.
- Inform palliative team of all new admissions and discharges.
- Coordinate referrals utilizing the referral process.
- Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

LICENSURE, CERTIFICATION, AND EDUCATIONAL REQUIREMENTS
- Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice
- A minimum of three (3) years of nursing experience
- Palliative care, hospice, or home health care experience is preferred
- CHPN (Certified Hospice and Palliative Nurse) is preferred
