Job Description:
Are you a dedicated LPN seeking an exciting opportunity to make a difference in patient care? We are urgently looking for a skilled and compassionate Triage Nurse to join our dynamic healthcare team. In this vital role, you will be responsible for assessing patient needs, providing guidance, and coordinating care to ensure the best possible outcomes for our patients.
Key Responsibilities:
Conduct thorough triage assessments for incoming patients, identifying urgent health concerns and prioritizing care needs. Utilize clinical judgment to determine the appropriate level of care and recommend next steps, including referrals, interventions, or follow-up appointments. Maintain accurate and detailed patient records, documenting assessments, treatments, and communications in a timely manner. Collaborate with physicians and other healthcare professionals to develop and implement patient care plans. Provide education and resources to patients and their families to promote understanding of health conditions and care options. Ensure compliance with all applicable healthcare regulations and organizational policies.
Qualifications:
Current LPN licensure in [State]. Proven experience in a triage or emergency care setting is preferred. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal abilities to interact effectively with patients and team members. Ability to prioritize tasks in a fast-paced environment while providing compassionate care.
