Job Description

Job Summary Join our dedicated healthcare team as a Chronic Care Management Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA). In this vital role, you will provide comprehensive support to patients with complex, chronic health conditions by coordinating care, conducting assessments, and facilitating communication between patients and healthcare providers. Your expertise will help improve patient outcomes, enhance care quality, and promote proactive management of chronic illnesses. This position will perform comprehensive assessment of high-risk patients. Collaborate with primary care providers to ensure the implementation of an individualized, comprehensive care plan with specific interventions designed to engage the patient. Address, identify, and continuously reassess cost‑efficient, appropriate levels of care. Keep detailed records of clinical, functional, and fiscal outcomes during the management process. Identify opportunities for health promotion and illness prevention. Prevent adverse patient occurrences when possible and intervene quickly if prevention is not possible, thereby minimizing poor outcomes. Educating patients is a required component of any successful CCM program. Core educational focus areas are as follows: personalized care plans, medication adherence, lifestyle modifications, and symptom management.

Wilson Medical Group is searching for a qualified and motivated Chronic Care Management LPN or Certified Medical Assistant to join our team. Clinical experience in an outpatient setting is desired. Familiarity with computers is imperative. The ideal candidate will be diligent, flexible, and able to work well in a team and individual environment.
Pay:
$20.00 - $35.00 per hour
