$100 start bonus Contact•14694894879 E-Mail•Job Summary We are seeking a dedicated and compassionate Local Licensed Practical Nurse (LPN) to join our healthcare team. The LPN will provide high-quality patient care within a variety of settings, including long-term care facilities, assisted living communities, and community health programs. This role involves working closely with patients of diverse backgrounds, including individuals with developmental disabilities, dementia, and chronic health conditions. Duties Perform patient assessments by monitoring vital signs, conducting physical examinations, and evaluating physiological responses to treatment. Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management. Assist with complex procedures such as catheterization, wound suturing, and sterile specimen collection while maintaining aseptic techniques. Manage patient care plans by coordinating with interdisciplinary teams, including physicians, therapists, and case managers. Document all patient interactions comprehensively in electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Provide specialized care for patients with memory impairments or neurological conditions such as Alzheimer's disease or traumatic brain injuries. Support acute pain management strategies and participate in triage processes for urgent medical needs. Requirements Valid LPN license in good standing with relevant state licensing boards. Prior experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice care, or outpatient clinics. Proficiency with EMR/EHR systems like Epic, Cerner, or Athenahealth; familiarity with ICD coding (ICD-9/ICD-10) is preferred. Ability to perform venipuncture, IV insertion, catheter care, and tube feeding procedures accurately. Strong communication skills for effective patient assessment and interdisciplinary collaboration.
