Shift:
11P - 7A Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic or Cerner. Administer medications and injections accurately, including IV infusions and catheterizations, following strict aseptic techniques and HIPAA regulations to ensure patient safety. Manage airway care, ventilator support, and acute pain management for patients in ICU or emergency room settings with Level I Trauma Center experience. Conduct diagnostic evaluations such as specimen collection, laboratory processing, sonography, and medical imaging procedures to support clinical decision-making. Provide specialized care for individuals with developmental disabilities, Alzheimer's disease, dementia, memory care needs, and behavioral health conditions. Assist in discharge planning, case management, and coordination of post-acute or home & community care services to ensure seamless patient transitions. Educate patients and families on health coaching topics like nutrition, medication administration, wound care (including suturing), and chronic disease management to promote ongoing wellness. Requirements Valid licensure as an LPN or RN with current state certification; experience working in diverse healthcare environments such as hospital medicine, medical-surgical units, or outpatient clinics preferred. Critical care experience including ICU or Level II Trauma Center exposure is highly desirable; familiarity with Level I Trauma Center protocols is a plus. Proficiency with EMR/EHR systems like Epic, Cerner, Athenahealth or eClinicalWorks; strong medical documentation skills are essential. Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures such as spinal taps or gastric lavage; understanding of anatomy and physiology to support diagnostic evaluation. Experience working with individuals with disabilities or in specialized areas such as hospice care, infectious disease care, or pediatric/neonatal ventilator management is advantageous. Ability to perform phlebotomy, venipuncture, tube feeding administration, sterile processing techniques, and use of medical equipment like ventilators or Hoyer lifts. Excellent patient assessment skills combined with the ability to work effectively in fast-paced emergency room or urgent care settings while maintaining compliance with HIPAA standards.
