We are seeking experienced and compassionate Licensed Practical Nurses (LPNs) to join our Monadnock Region home care team on a per diem basis. This position offers independence, flexibility, and meaningful one-on-one care for patients who prefer to remain in the comfort of their homes throughout Keene and the surrounding communities. Position Overview The LPN will provide skilled nursing visits focused on promoting recovery, maintaining stability, and enhancing quality of life. This is an ideal role for nurses who thrive in autonomous settings and want flexible scheduling with competitive per-visit compensation. Key Responsibilities- Conduct in-home patient assessments, medication management, and wound care.
- Monitor and document changes in patient condition and report to supervising RNs or physicians.
- Educate patients and caregivers on treatment plans, medications, and home safety practices.
- Maintain accurate, timely documentation using electronic charting.
- Collaborate with multidisciplinary care teams to ensure coordinated and effective patient care. What We Offer
