Requirements:
- Valid Nursing licensing
- LPN in the State of NJ
- Experience in community nursing
- Valid NJ Driver's License with good driving record
- Criminal Background Clearance
- Lifting or assist in lifting disabled adults as medically necessary
- Provides on-call support as determined by the program needs
- Proficient in MS Office applications
Responsibilities Include:
1. Successfully complete and demonstrate proficiency in all areas of required training including Overview of Developmental Disabilities, Abuse and Neglect, Medication Administration, and CPR/First Aid. 2. Provide nursing coordination for individuals that meet their health care needs through periodic home visits. 3. Function as a liaison and advocate for the individuals when dealing with physicians, psychiatrists and consultants as needed. 4. Provide on-going health care training to staff. 5. Implement and train employees on new and revised health related policies and procedures in a timely manner. 6. Participate in regular pharmacy reviews with the consulting pharmacist and follow up as needed. 7. Audit medication administration in the homes. 8. Complete daily, weekly, monthly, quarterly and yearly paperwork and assessments of individuals' health care needs. 9. Ensuring that staff have called 911, respond by phone to medical calls of concern within 10 minutes of being called. 10. Monitor the medical files of individuals to ensure that all documentation is present and correct. This includes quality reviews of the MAR and medication storage. 11. Participate in IHP meetings, IDT meetings, staff meetings and other meetings as assigned. 12. Provide medication training and other state required healthcare related training as required for agency staff. 13. Order medical supplies on a monthly basis for all community residences. 14. Perform health screenings and medical assistance for assigned programs. 15. Comply with all standards to assure the health and safety of all staff/individuals we serve. 16. Report unusual incidents and complete internal incident reports as needed according to MAS procedures. 17. Maintains effective relationships and open communication with Midland staff, service recipients, visitors and community members. 18. Performs other related duties, special assignments and projects, as required. Remains flexible and adaptable in work schedule as defined by organizational needs. 19. Abides by the Midland Mission Statement in promoting ethical behavior, establishing relationships and providing guidance in decision-making situations. 20. Remains current with required training certifications, meets New Jersey-regulated licensing and regulatory accreditations, and adheres to mandatory requirements. 21. Maintains effective verbal and written communications with colleagues, those served, and their family members and/or guardians when applicable. 22. Reports unusual incidents and completes internal incident reports as needed according to MAS procedures. 23. Willingly and effectively cooperates with Midland, The Department of Human Services, The Office of Licensing, and any other licensing or state agency or local municipalities in any inspections and investigations, upon request.Job Type:
