LPN Licensed Practical Nurse Job Summary:
Silver Healthcare is seeking a dedicated and compassionate LPN Licensed Practical Nurse to join our team at the Silver Health Care Center in Cherry Hill, New Jersey. Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs) Monitor and report changes in resident condition to the nursing team, ensuring timely interventions and effective communication Collaborate with interdisciplinary teams to develop and implement individualized care plans, promoting resident autonomy and dignity Participate in quality improvement initiatives, incorporating evidence-based practices to optimize patient outcomes Maintain accurate and detailed records of patient care, adhering to regulatory requirements and organizational standards Demonstrate a strong commitment to patient-centered care, prioritizing resident needs and preferences About the
