LPN Licensed Practical Nurse Job Summary:

We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our dedicated team of healthcare professionals at Silver Healthcare in Cherry Hill Township, New Jersey. Responsibilities:

Provide direct patient care to residents in our long-term skilled nursing and short-term rehabilitation programs Administer medications, vaccinations, and treatments as prescribed by physicians Monitor vital signs and report any changes to the nursing team Develop and implement individualized care plans to meet the unique needs of each resident Communicate effectively with residents, families, and other healthcare professionals to ensure seamless care transitions Participate in interdisciplinary team meetings to discuss resident progress and develop care strategies Maintains accurate and up-to-date records of patient care and treatment

Qualifications:

Current LPN license in the state of New Jersey Basic Life Support (BLS) certification Ability to lift, transfer, and reposition patients as needed Excellent communication and interpersonal skills
