Job Description

Our full-time Licensed Practical Nurse (LPN) works under the direction of the Staff Nurse and the attending physician. The nurse provides skilled nursing in order to maximize the comfort and health of patients and their families. This position covers patients in the Morris County region . Responsibilities for a Licensed Practical Nurse (LPN) in home care are: Provides direct nursing care to clients in their place of residence. Supplements the nursing care needs of the patient as provided by the RN and as specified in the plan of care including: Providing services in accordance with organizations policies and procedures and regulations that define scope of practice. Prepares clinical and progress notes documenting outcomes of interventions. Prepares equipment and materials as needed according to agency policy Provides follow‑up visits which meet patient needs and conform to physician ordered frequency. Assists the patient in learning appropriate self care techniques Evaluate patients/families response to care Ensures communication of information to appropriate team members Follow implementation of the plan of care including specialized duties related to patient needs, with the client, physician, and family. Serves as liaison between client/physician, client/case manager, client/family, and client and other community resources. Is accountable for client records including: Medical record: Which is updated during each visit with the client and includes all pertinent data from assessment, referral to other professionals, dates of physician visits, emergency contacts, and procedures.
Narrative record:
which is completed for each visit to describe unusual findings, unstable conditions or any unusual circumstances and any physician contacts occurring via telephone with signed and dated entries. Medications with side effects.Financial records:
Validating current fee sources and recording appropriate numbers.Verbal orders:
Which are confirmed with an RN; entered into the client record and electronically reviewed and approved by an RN; submitted to be signed by the attending physician. Discharge Planning withClient:
Under direction of RN and according to plan of care and Agency procedure. Participates in Conferences with case manager, therapy and ancillary staff as outlined in Agency policy. Conferences may cover client topic/problems the nurse is encountering. Participates in professional development and activities which will increase knowledge of community nursing. Completes all mandatory in-services and attends mandatory meetings. Maintains current knowledge and understanding of Federal/State regulations and facilitates proper utilization of health care resources. Meets Agency standards in demonstrating competency in clinical practice. Represents the Agency in the home and community in a professional manner. Qualifications for a Licensed Practical Nurse (LPN) in home care are: Current licensure as a Licensed Practical Nurse (LPN) in the State of New Jersey. The VNANNJ is anEEOE/M/F/V/D
