This monitoring and evaluation should always comply with all state, federal and current Agency policy. The LPN will provide medical supervision for individuals in consultation, as needed, with physicians and allied health care professionals. The LPN will assist the Nursing Supervisor in the development and implementation of preventative health services for the individuals that the Agency serves. Essential Duties and Responsibilities include the following. Other duties may be assigned. Assesses the on-going health and/or medical needs of the individuals of our various programs and reports to the Nursing Supervisor. Assures that all identified health needs are met through the coordination of service delivery by external providers and, where indicated, consultation with providers and Agency staff. Maintains and/or monitors the maintenance of all necessary records, reports and individual files as related to the health needs, services and medications of individuals served by the Agency. Documents medical progress and concerns in accordance with professional nursing knowledge and standards. Educates Direct Support Professionals (DSPs) about relevant signs and symptoms in relation to our individuals' healthcare and medical needs. Provides continuing education regarding the individuals' medications, potential medication reactions, side effects and interactions. Assists the Nursing Supervisor with developing and facilitating health-related in-service programs or various topics regarding any specialized medical care for our individuals. Works in conjunction with physical, occupational, and speech therapists to evaluate restorative and or habilitation potential. Assists program management with acquisition, evaluation and training regarding adaptive equipment, mobility aids and medical equipment. Attends individuals at hospitals and skilled nursing facilities when they are admitted. Assists the Nursing Supervisor and program management in coordinating healthcare discharge planning when an individual is hospitalized and/or in any in-patient rehabilitative/skilled nursing care facilities. Attends and participates in all meetings pertaining to individuals' assigned (IDT/ISP/NJCAT) meetings and/or re-assessments and program staff meetings. Attends individual in-patient/out-patient care meetings if needed. Maintains a weekly schedule which is reviewed by the Nursing Supervisor. Rotates and provides 24/7 on-call nursing support for individuals' medical needs. This on-call rotation includes nights and weekends. This position will require an Agency-provided on-call cell phone to be used at all times during the rotation and all scheduled working hours. All concerns will be reported to the Nursing Supervisor. Provides on-call assessment and treatment 24/7 (including nights and weekends) when on call, at the individuals' locations, which may include Agency residences, hospitals, or other medical providers. Accompanies individuals and staff to medical appointments, as deemed necessary. Assists with seeking out and linking individuals to community health care as needed. Acts as Agency medical professional regarding OSHA Blood Borne Pathogens policy and procedures. Maintains communication with CLA programs to keep current on the medical needs of the individuals. This will include program visits, telephone calls, emails and THERAP documentation. Assists the Nursing Supervisor with routine medical quality assurance checks on all CLA programs. Attends nursing workshops/conferences/meetings as deemed appropriate by the Director of CLA and/or the Nursing Supervisor to maintain continued nursing education and successfully complete all agency pre-service trainings. Cooperates with the Agency, the Division of Developmental Disabilities (DDD) and all other state and federal entities regarding any inspections and investigations. Complies with all Agency policies and procedures. Follows all NJ Board of Nursing laws, rules, standards, and regulations. Follows all policies governing the practice of nursing. Follows the NJ Board of Nursing Practice and Decision-Making Models. Maintains a current license to practice nursing and completes all required CE credits to maintain an active license. Assumes additional duties, as assigned, by the Director of CLA and the Nursing Supervisor. Submits to a criminal background check, and is determined to be not disqualified by the Department of Human Services, prior to working with the individuals served by the Agency.
