The ideal candidate will have a strong background in nursing and a passion for providing high-quality care to residents in various settings, including nursing homes, assisted living facilities, and memory care units. This role involves working closely with patients, families, and interdisciplinary teams to ensure optimal health outcomes and enhance the quality of life for our residents.

Requirement:
Must have 1+ year of LPN LTC experience RN License AHA BLS Covid card Duties Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs. Assist with daily living activities such as bathing, dressing, and feeding residents. Conduct thorough patient assessments and document care provided in compliance with HIPAA regulations. Collaborate with healthcare teams to develop and implement individualized care plans. Educate patients and families on health management, medication administration, and disease prevention. Maintain accurate medical records and ensure proper documentation of patient interactions. Participate in quality improvement initiatives to enhance patient care services. Support residents with Alzheimer's care, dementia care, and other specialized needs.
