Position Overview As a member of Travis's support team, the LPN will provide skilled nursing services, personal care support, health monitoring, and community-based assistance while promoting safety, dignity, independence, and overall well-being. Travis is diagnosed with a developmental disability and utilizes adaptive equipment to support his mobility. The ideal candidate will be patient, attentive, physically capable of providing hands-on assistance, and committed to fostering a supportive and empowering environment. Nursing Responsibilities Clinical Care & Health Management Provide direct nursing care and ongoing health monitoring Administer medications and treatments as prescribed Monitor health status and respond appropriately to changes in condition Complete medication reconciliation and nursing assessments Document all assessments, interventions, observations, and clinical decisions accurately and timely Maintain compliance with physician orders, Individual Support Plans (ISP), and agency requirements Conduct comprehensive assessments and develop person-centered nursing care plans Utilize clinical judgment and critical thinking to support positive health outcomes Report concerns promptly and communicate recommendations for resolution Educate and support family members regarding health-related needs and wellness goals Care Coordination Collaborate with physicians, behavioral health providers, therapists, family members, and support staff Ensure effective coordination of services and continuity of care Identify appropriate resources and referrals to meet individual needs Participate in interdisciplinary planning and quality improvement initiatives Staff Supervision & Training Serve as Clinical Nursing Supervisor for Direct Support Professionals (DSPs) Conduct supervisory visits in accordance with regulatory requirements Provide guidance, support, and clinical oversight to direct care staff Participate in staff evaluations and ongoing training initiatives Compliance & Professional Standards Maintain knowledge of current nursing standards, best practices, and regulations Adhere to organizational policies and procedures Ensure compliance with New Jersey Department of Developmental Disabilities (DDD) requirements Complete assigned responsibilities independently and within established timelines Demonstrate professionalism, integrity, and organizational values in all interactions Personal Care & Daily Living Support Assist with activities of daily living, including: Personal hygiene Dressing and grooming Feeding assistance Toileting support Mobility assistance Assist with safe transfers and adaptive equipment usage Promote dignity, comfort, independence, and self-determination Maintain a safe, clean, and supportive environment Anticipate needs and provide proactive support Community Engagement & Enrichment We are looking for someone who enjoys being active, social, and involved in the community.
