Job Description

Position Summary Brookwood Home Health is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN). The Home Health LPN is responsible for delivering quality patient care, monitoring patient conditions, supporting treatment plans, and promoting patient independence while maintaining compliance with state and federal home health regulations. Essential Duties and Responsibilities Provide skilled nursing services to patients in accordance with physician orders and individualized care plans. Monitor and document patient health status and report changes or concerns to the supervising RN and physician. Administer prescribed medications and treatments safely and accurately. Perform skilled nursing procedures including wound care, catheter care, injections, dressing changes, and vital sign monitoring. Assist in implementing and updating patient care plans. Educate patients and caregivers regarding medications, treatments, disease processes, safety precautions, and home care needs. Maintain accurate, timely, and complete clinical documentation in accordance with agency policies and regulatory standards. Observe infection control and safety procedures at all times. Participate in interdisciplinary care team meetings and case conferences as needed. Provide compassionate care while respecting patient rights, dignity, and confidentiality. Travel to patient homes within assigned service areas. Qualifications Current Licensed Practical Nurse (LPN) license in the State of New Mexico. Graduate of an accredited practical nursing program. Minimum one (1) year nursing experience preferred. Home health or community-based care experience preferred. Current CPR certification required. Valid driver's license, reliable transportation, and current automobile insurance. Strong clinical assessment and communication skills. Ability to work independently and manage time effectively. Basic computer and electronic documentation skills required. Physical Requirements Ability to lift, reposition, and assist patients as needed. Ability to stand, walk, bend, stoop, and reach throughout the workday. Ability to travel between patient homes and work in varying home environments. Ability to use standard nursing and office equipment. Work Environment Patient homes and community-based settings. Exposure to varying environmental conditions and potential hazards. Flexible schedule that may include evenings, weekends, or on-call responsibilities.
Pay:
$31.34 - $37.74 per hour
