Administer medications and treatments as prescribed. Monitor, assess, and document resident conditions. Collaborate with RNs, physicians, and the interdisciplinary care team. Maintain accurate clinical documentation. Follow facility policies, safety standards, and infection control guidelines. Requirements Active LPN license required. Previous Long-Term Care (LTC) experience preferred. Strong communication and teamwork skills. Ability to work 12-hour day or night shifts.
