Our established Medicare-certified home health agency is seeking an experienced Assistant Director of Nursing (ADON) to join our leadership team. This position supports a skilled census of approximately 200 patients and plays a vital role in maintaining exceptional patient care, regulatory compliance, operational efficiency, and agency growth. We are looking for an experienced RN or LPN/LVN who enjoys leading people, solving problems, building structured systems, and helping teams succeed. The ideal candidate understands that excellent communication, accountability, organization, and consistency lead to outstanding patient outcomes. This position includes office leadership responsibilities as well as occasional field visits to support patient care when needed. Responsibilities Assist the Director of Nursing with daily clinical operations. Ensure compliance with Medicare Conditions of Participation (CoPs), CMS regulations, state regulations, accreditation standards, and agency policies. Assist with clinician scheduling to ensure timely patient visits and efficient utilization of staff. Monitor staffing levels and coordinate coverage for patient care needs. Track, monitor, and manage patient referrals from receipt through admission, ensuring prompt follow-up with referral sources, referral qualification, insurance verification, clinician assignment, and timely patient admissions. Monitor referral activity, pending admissions, referral conversion rates, and census trends while identifying opportunities to improve operational efficiency and agency growth. Develop and maintain strong working relationships with physicians, hospitals, case managers, discharge planners, assisted living facilities, and other referral sources. Review OASIS assessments and clinical documentation for accuracy, timeliness, and Medicare compliance. Assist with quality assurance, performance improvement, infection prevention, and survey readiness activities. Mentor, coach, and support nurses, therapists, and clinical staff. Participate in supervisory visits, patient admissions, skilled nursing visits, and field visits as needed. Assist with investigating patient concerns, incident reporting, and clinical problem-solving. Help develop and maintain structured workflows that create predictable, measurable outcomes and operational excellence. Promote a positive, professional, team-oriented work environment focused on accountability and exceptional patient care. Qualifications Current New Mexico RN or LPN/LVN license (or compact equivalent) in good standing (Required) Minimum three (3) years of recent Medicare-certified home health experience (Required). Previous home health management, clinical supervisor, or leadership experience (Required). Strong working knowledge of Medicare home health regulations, Conditions of Participation, and OASIS documentation (Required). Experience with home health clinician scheduling and daily clinical operations (Required). Experience managing clinical teams and operational workflows (Strongly Prefered) Demonstrated experience tracking referrals, monitoring admissions, managing referral pipelines, and maintaining strong relationships with referral sources. Excellent communication, leadership, organizational, and critical thinking skills. Strong attention to detail and ability to manage multiple priorities in a fast-paced environment. Current BLS (Required) Experience with electronic medical records (EMR) (Kinnser-Wellsky preferred) and Microsoft Office. Valid driver's license, reliable transportation, and current automobile insurance (Required). Must pass a Criminal Background check (Required). Preferred Qualifications Experience with Medicare surveys and survey preparedness. Experience improving operational efficiency and clinical workflow. Knowledge of quality metrics, hospitalization reduction strategies, and performance improvement initiatives. Experience managing clinical teams and operational workflows (Strongly Preferred) Experience improving census growth and staff retention We're Looking for Someone Who Leads by example.
