LPN LONG TERM CARE

Hours Days Shift Details:
6.45 AM to 7.15 PM 48 hours/week. (4
  • 12) 14-week contract Deliver comprehensive nursing care to residents in a long-term care facility, including medication administration, wound care, and vital sign monitoring.
Assist with patient assessments, documenting health status and reporting changes promptly to the healthcare team. Administer IV infusions and injections while adhering to aseptic techniques and safety protocols. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to residents' needs. Support residents with activities of daily living, including mobility assistance, hygiene, and nutrition management. Maintain accurate medical records using Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Participate in discharge planning, family communication, and case management activities to promote continuity of care. Follow infection control procedures diligently and contribute to maintaining a safe environment for residents and staff. Qualifications Valid state licensure as a Licensed Practical Nurse (LPN) with current certification. Prior experience working in long-term care facilities or nursing homes is highly preferred. Demonstrated proficiency with EMR/EHR systems such as Epic, Cerner, or Athenahealth. Knowledge of medical terminology, physiology, and diagnostic evaluation techniques relevant to geriatric and chronic care populations. Experience with IV infusion therapy, medication administration, and basic life support (BLS). Familiarity with dementia care, Alzheimer's disease management, and memory care practices. Strong communication skills coupled with the ability to work collaboratively within multidisciplinary teams. Ability to handle emergency situations effectively, including triage and acute pain management scenarios.
