Licensed Practical Nurse - LVN

LPN Full Time and Part Time AM and NOC available Sunset Ridge Post Acute is seeking a compassionate, competent, and committed Licensed Practical Nurse (LPN to join our Skilled Nursing and Post-Acute Care team. Requirements & Qualifications Active state LVN license Current CPR or BLS certification Wound care certification is a plus Knowledge of medical and hospital terminology Ability to maintain professional relationships with residents, families, and staff Strong commitment to confidentiality and compliance with company policies Completion of mandatory Code of Conduct and compliance training Prior experience in SNF or post-acute care preferred Key Responsibilities Record patient history and monitor vital signs (blood pressure, temperature, weight) Perform basic wound care, including cleaning and bandaging Administer injections and monitor medication responses Document patient information accurately in electronic medical records Monitor fluid and food intake/output Assist residents with daily living activities, including bathing and dressing Safely move and transfer residents as needed Observe and support residents' mental health and emotional needs Maintain a safe environment and follow infection prevention guidelines Communicate changes in resident condition to nursing staff promptly Physical Requirements (With or without reasonable accommodation) Perform essential nursing duties including lifting, moving, and assisting residents Frequent standing, walking, bending, reaching, and occasional lifting Ability to manage the physical, mental, and emotional demands of caring for medically complex or elderly residents We are an Equal Opportunity Employer .
