Job Description

We're a warm and welcoming home health agency hiring for a full-time Licensed Practical Nurse, LPN who believes healing happens best at home - and you know how to make someone smile even during wound care - you're our kind of person. Home Health experience is a plus, but not required. Position Summary The Licensed Practical Nurse (LPN) provides skilled nursing care to patients in their place of residence under the direction and supervision of a Registered Nurse (RN) and in accordance with the physician's plan of care. The LPN ensures high-quality, safe, and compliant care delivery while supporting the agency's mission, regulatory requirements, and clinical standards. Essential Duties & Responsibilities Clinical Care & Patient Services Provide direct nursing care to patients in their home according to the established plan of care. Perform skilled nursing procedures within scope of practice including but not limited to: Vital signs monitoring and assessment Medication administration (oral, injectable, topical) Wound care and dressing changes Catheter care and ostomy care Diabetic care and blood glucose monitoring Observe, document, and report patient condition, symptoms, and response to treatment to supervising RN. Identify changes in patient condition and initiate appropriate clinical interventions. Provide patient and caregiver education on disease processes, medications, infection prevention, and safety. Documentation & Compliance Accurately complete all required documentation. Maintain timely clinical notes, visit documentation, and communication logs. Participate in care coordination and interdisciplinary team meetings. Follow infection control policies, patient safety standards, and HIPAA regulations at all times. Professional Collaboration Communicate effectively with physicians, RNs, therapists, case managers, patients, and family members. Support quality improvement initiatives and performance outcomes. Comply with all corporate policies, clinical protocols, and accreditation standards (CMS, ACHC, Joint Commission, etc.). Qualifications & Requirements Education & Licensure Current, active LPN license in Nevada Current CPR certification. Experience Minimum of one (1) year of nursing experience preferred. Home health or community-based care experience a plus. Knowledge & Skills Strong clinical assessment and nursing skills. Knowledge of home health regulations, patient rights, and infection control standards. Excellent documentation and time management abilities. Strong interpersonal and communication skills. Ability to work independently in a home-based setting. Physical & Environmental Requirements Ability to lift, reposition, and transfer patients. Ability to travel to patient homes within assigned service area. Ability to stand, walk, bend, and kneel for extended periods. Visual and auditory acuity sufficient to perform clinical assessments and documentation. Compliance & Ethical Standards Maintains patient confidentiality and professional boundaries. Adheres to ethical standards of nursing practice. Complies with all federal, state, and agency regulatory requirements. Performance Expectations Consistently delivers safe, high-quality patient care. Maintains accurate and timely documentation. Demonstrates professionalism and compassion in all interactions. Supports agency goals related to patient outcomes, quality measures, and regulatory compliance.
