The LPN with CDPCs Community Residence with provide companionate care and patient-centered and recovery-focused mental health services to individuals with severe and persistent mental illness, including: Performing oversight of medical needs Coordinating care with psychiatric and medical providers Ordering medication Administering medication and teaching medication independence skills CDPC, a Joint Commission accredited Office of Mental Health (OMH) facility, provides inpatient and outpatient treatment and rehabilitation to adults who have been diagnosed with serious mental illness, outpatient treatment services for children and adolescents diagnosed with serious emotional disturbances, and operates adult community residences in the Albany/Capital District area. CDPC is committed to supporting underserved individuals, organizations, and communities along with activities and initiatives to reduce disparities in access, quality, and treatment for underserved populations. Minimum Qualifications:
