LPN (Licensed Practical Nurse) Full Time 3:00 pm - 11:00 pm

Seeking a knowledgeable, professional and diligent LPN to act as a house supervisor on the overnight shift ( 3:00 pm - 11:00 pm ). Candidate must be willing to lead by example, motivate others and demonstrate excellent clinical judgement.

Responsibilities:

Monitoring all assisted living residents each and every shift for clinical needs, change in condition, hospice pain management, etc. Documentation of all clinical care, medication administration and treatments per MD order. Supervise overnight facility staff Provide excellent report to oncoming shift in regard to resident status and any overall facility concerns. Administers medications or assists residents in the self-administration of medications in accordance with professional practice standards, Assisted Living policy, and NYS Licensing Regulations. Complies with NYS Department of Health Assisted Living policies and procedures related to controlled substances, narcotic count, and documentation. Communicates professionally and timely with other health professionals including but not limited to MD offices, hospital care managers, nursing home social services staff, and pharmacies. Implements, monitors and supervises staff providing care in accordance with resident care plans in consultation with the RN. Coordinates with Social Workers and/or Case Managers, RN and Director of Wellness Department to encourage participation of residents, family members and guardians in decision making, planning for and meeting resident needs.

License Requirements:

Current NYS LPN License Required Experience in long term care / Assisted Living setting Must possess a high degree of interpersonal communication skills to relate with a wide variety of individuals. Must have demonstrated leadership skills and the ability to exercise sound judgment. Must possess strong organizational and time management skills.
