Job Description
Position Overview United Health Services (UHS) is seeking a Licensed Practical Nurse (LPN) to join our dedicated care team in Candor, NY. In this role, you'll deliver direct patient care, support daily clinical operations, and help ensure a safe and compassionate environment for those we serve. Primary Department, Division, or Unit:
UHS Primary Care Candor Work Shift and Schedule:
This is a per diem position, which means you will work on an as needed, agreed upon basis. Working hours will be assigned by your manager.Compensation Range:
$22.97•$34.46 per hour, depending on experience This position is not eligible for benefits.•Job Responsibilities:
Perform clinical tasks such as taking vital signs, collecting specimens, and assisting with procedures, always within the LPN scope of practice and under supervision when required Administer medications safely and accurately, utilizing knowledge of pharmacology and following UHS policies and current clinical standards Educate patients and their families on health promotion, disease prevention, and management strategies, as directed by a provider or registered nurse Support and ensure compliance with all regulatory standards including Joint Commission, NYS Department of Health, and infection control protocols Perform CLIA-waived laboratory testing, including quality control procedures, documentation, and participation in tri-annual AAFP proficiency testing Maintain accurate and timely documentation in the electronic medical record system to support patient safety and care coordination Participate in department-specific credentialing and lab validations as required by practice standards and regulations Collaborate with providers, RNs, and the broader care team to coordinate patient care and promote positive health outcomes Assist with front office functions such as scheduling, patient intake, and communication of clinical instructions when appropriate Actively engage in Performance Improvement (PI) initiatives by reviewing data, offering feedback, and implementing changes to enhance care quality Maintain clinical competency by participating in ongoing training, in-services, and educational opportunities Uphold a culture of patient-centered care by contributing to a safe, respectful, and welcoming environment for all individualsPosition Qualifications:
Minimum Required:
Graduated an accredited school of Practical Nursing Current New York State Practical Nurse License, a Limited Permit, or ability to practice within the 90 day grace period allowed by New York State guidelinesPreferred:
