Seeking dedicated Licensed Practical Nurse's for Full Time, Part Time, or Per Diem shifts at our 31-bed intermediate care facility in Commack.
Schedule:
7:00am-3:30pm, 3:00pm-11:30pm, or 11:00pm-7:30am •All F/T and P/T LPN shifts at this location require at least one weekend day per week •General Purpose of the Job Under the supervision of a Registered Nurse, assist in the overall health and safety of the individuals served. Essential Duties and Responsibilities include the following. Other duties may be assigned. Knowledge of all medications for all individuals. Administer medications and treatments as prescribed by physician. Generate incident, seizure, and illness reports. Monitor and report health care on daily flow report records. Instruct mid-level supervisors and direct care staff in above, i.e., hygiene, diet, weights, etc. Submit written monthly, quarterly, annual nursing/medical progress notes. Coordinate laboratory care and medical appointments (routine, fees, special and emergency). Maintain functioning interpersonal relationships with other disciplines. Maintain ongoing communication with other programs, i.e., client health care. Monitor program participants during mealtimes. Order, record, and maintain sufficient/adequate supply of medication. Maintain sufficient/adequate amount of medical supplies/materials. Maintain accurate medical records including medication administration sheets, Kardex, Formulary, etc. Provide emergency first aid. Coordinate emergency services. Supervise direct care staff in absence of mid-level supervisor. Education and/or Experience Associate's degree (A. A.) or equivalent from two-year college or technical school and one to six months' related experience and/or training; or equivalent combination of education and experience.
