Job Summary We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care in a Family Practice setting, ensuring the well-being of patients. Responsibilities Face-to-face & telephonic communication Perform phone triage, patient education and coaching Use of disease management interventions to engage patients in their health care e-prescribe Immunizations EMR experience Vital signs Medication reconciliation Specimen collection with in-house testing EKGs and spirometry Participate in the development and implementation of individualized care plans. Skills Excellent communication skills for effective interaction with patients, families, and team members. Ability to work collaboratively within a team environment. Strong organizational skills with attention to detail in clinical documentation. Compassionate approach to patient care with a focus on empathy and support.
