10:45 PM•7:15 AM (30-minute unpaid meal break) Position Summary We are seeking compassionate and dependable Licensed Practical Nurses (LPNs) to provide quality nursing care in a long-term care environment. Under the direction of the interdisciplinary care team, the LPN is responsible for administering medications and treatments, monitoring resident conditions, documenting care, and ensuring compliance with established nursing standards and regulatory requirements. Essential Duties and Responsibilities Administer medications and treatments in accordance with physician orders and nursing standards. Assess and monitor resident conditions and promptly report significant changes to the appropriate healthcare provider. Accurately document nursing care, treatments, and interventions. Supervise and provide direction to assigned Certified Nurse Aides (CNAs). Assist with resident admissions, transfers, and discharges. Participate in interdisciplinary care planning and resident assessments. Promote resident safety, dignity, privacy, and infection prevention practices. Ensure compliance with facility policies, state and federal regulations, and professional standards of practice. Collaborate effectively with physicians, nurses, therapists, and other members of the healthcare team. Maintain accurate records and support quality assurance initiatives. Minimum Qualifications Graduate of an accredited Practical Nursing Program. Active and unrestricted New York State Licensed Practical Nurse (LPN) license. Current CPR certification required. Minimum one (1) year of nursing experience preferred. Long-term care experience preferred. Strong clinical judgment, communication, and organizational skills. Ability to work effectively in a team-oriented environment while delivering compassionate, resident-centered care. Credentialing and Compliance Requirements Candidates must provide: Current and unrestricted New York State LPN license with renewals as applicable. Current CPR certification. Current resume. Two (2) professional references from current or previous employers verifying dates of employment and satisfactory performance. Physical examination and health clearance confirming fitness for duty. Proof of immunity to Rubella and Rubeola. Compliance with applicable
COVID-19
vaccination requirements. Documentation of an annual PPD skin test for tuberculosis; candidates with a known positive PPD must provide a negative chest X-ray completed within the past two years. Copies of applicable diplomas, certifications, and professional training records. Schedule and Work Requirements Eight-hour shifts available on days, evenings, and nights. Overtime opportunities may be available and must be approved in advance. Holiday shifts may be required and are compensated in accordance with applicable policies. Candidates must demonstrate excellent attendance, punctuality, and reliability. Ability to fulfill scheduled assignments and provide high-quality nursing care in a long-term care setting.
Benefits Pay:
$28.00•$32.00 per hour
