Role:

On Site Role & Responsibilities:

Provide direct nursing care to residents in a Long-Term Care (LTC) / Skilled Nursing Facility (SNF) setting. Administer medications and treatments in accordance with physician orders, facility policies, and New York State regulations. Accurately document resident care and clinical activities in the facility's electronic medical record system (Point Click Care). Monitor resident conditions and report changes in status to supervising Registered Nurses and physicians as appropriate. Maintain compliance with nursing standards of practice and professional code of ethics. Participate in resident care planning and interdisciplinary team activities. Provide leadership and direction to Certified Nurse Aides (CNAs) as assigned. Assist with unit operations and ensure continuity of resident care. Perform CNA-level duties during staffing emergencies as directed by facility leadership. Attend and successfully complete facility orientation and competency requirements. Work assigned shifts, weekends, holidays, and overtime as approved by the facility.

Mandatory Requirements:

Current New York State Licensed Practical Nurse (LPN) License. BLS Certification. About our
